ONC Certification
Theoria Medical ChartEasy™ is an ONC certified electronic health record (EHR) specifically designed for the post-acute and primary segment of care. ChartEasy™ is a secure, HIPAA compliant, cloud-based platform supported for web and iOS applications to meet the needs of all users. Theoria's EHR is designed to maximize provider efficiency and to ensure continuity of care and health equity for patients being cared for in skilled nursing facilities, home health settings or by their primary care physician. Theoria's technology team has self-developed and certified this software, optimized directly for its provider group, with maximized interoperability and integrations between facility partner EHRs and laboratory partner LIMS platforms.
This Health IT Module is 2015 Edition compliant and has been certified by an ONC-ACB in accordance with the applicable certification criteria adopted by the Secretary of Health and Human Services. This certification does not represent an endorsement by the U.S. Department of Health and Human Services.

Our Electronic Health Record system, ChartEasy™, is designed in alignment with the ONC Health IT Certification 2015 Cures Update criterion 170.315(b)(10) pertaining to EHI Export.
Patients can readily export their EHI in the following ways:
1. Direct Patient Access via ChartEasy Patient Portal: Theoria offers patients direct access to their EHI via the ChartEasy Patient Portal. This portal is designed to be user-friendly and secure, ensuring that patients can conveniently retrieve their information whenever needed. Records may be exported as both PDF and CCDA format.

2. Request to Theoria Medical Records Team: In instances where further assistance or specific record retrieval is required, patients may forward their request to Theoria's dedicated medical records team.
